Percutaneous Epididymal Sperm Aspiration (PESA)

Percutaneous Epididymal Sperm Aspiration (PESA) is a minimally invasive sperm retrieval technique used for men with obstructive azoospermia. This procedure involves aspirating sperm directly from the epididymis, a structure that stores and transports sperm from the testicles.

At DYU Advanced Fertility & Smart IVF Center, we provide state-of-the-art PESA procedures to help couples achieve parenthood through Intracytoplasmic Sperm Injection (ICSI) and In Vitro Fertilization (IVF).

Who Needs PESA?
  • Men diagnosed with obstructive azoospermia due to:
    • Vasectomy or failed vasectomy reversal
    • Congenital absence of the vas deferens
    • Infections or blockages affecting sperm transport
  • Patients requiring surgical sperm retrieval for ICSI treatments
  • Men who prefer a less invasive option compared to open surgical sperm extraction

The PESA Procedure

Step 1: Preparation
  • The procedure is performed under local anesthesia or mild sedation.
  • A fine needle is inserted into the epididymis to aspirate sperm-containing fluid.
Step 2: Sperm Retrieval
  • The aspirated fluid is processed in the laboratory to extract viable sperm.
  • If live sperm is found, it can be used immediately for ICSI or IVF, or frozen for future use.
Step 3: Post-Procedure Care
  • Patients can resume normal activities within 24–48 hours.
  • Mild discomfort or swelling may occur but typically resolves quickly.

Benefits of PESA
  • Minimally invasive with no surgical incisions
  • Quick recovery and minimal discomfort
  • High success rates for sperm retrieval in obstructive azoospermia cases
  • Can be performed alongside female partner’s egg retrieval for IVF
  • Option to freeze sperm for future fertility treatments
